WhatsApp is scanning photos in user reports and public profile images, not local photos pre or post upload/send.

Yes, and on-device CSAM scanning + E2EE iCloud is a strictly more private and mass-surveillance robust system than the status quo.

if they are, how did they only report ~200 images last year? My read was that they very much have not been doing this proactively, and the handful of cases were literally opportunistic and largely accidental/incidental to repairs.

Same. No where near the scale that FB/WhatsApp does, although they have reporting mechanisms too.
Which is precisely why this is wildly disingenuous from Apple and intended to progress to scanning E2E messages. There's no reason to do this on the user's phone when they can already do it in their own cloud!
